1. "Is It Working?" — No Visual State While AI Processes

This is the single most common AI UX failure we find. A user clicks a button, the AI starts processing, and the interface gives them nothing. No spinner. No progress. No indication that anything is happening at all.

What we see in real products:

A live event moderation tool where clicking "AI Reply" on an audience question shows no feedback while the model generates a draft. The moderator, under time pressure during a live event, clicks the button again. And again. Is it broken? Is it thinking? There is no way to tell.

A brain dump tool where users dictate unstructured thoughts and AI organizes them into tasks. After clicking "Organize", the panel sits static with no loading indicator, no "working on it" message. Users stare at a screen that looks identical to the screen before they clicked.

A content generation tool where "Generate article" triggers a process that takes 10 to 30 seconds. During that time, the interface shows "Generating article..." in small text but provides no cancel button, no progress estimate, and no indication of what will happen when it finishes.

Why this matters:

AI processing times are non-deterministic. Unlike loading a database record (predictable, fast), generating AI output can take 2 seconds or 45 seconds depending on complexity. Users have no mental model for how long to wait. Without feedback, they assume the product is broken within 3 to 5 seconds.

How to fix it:

  • Show an immediate state change when the user triggers AI (within 200ms). Even a subtle shimmer or "thinking" animation is better than nothing.
  • If the process takes more than 3 seconds, show estimated time or at least a reassuring message: "This usually takes 10 to 20 seconds."
  • Always provide a cancel button. Users need an escape hatch.
  • If your AI streams output (like an LLM), stream it to the user. Watching text appear token by token is vastly better than waiting for a completed block.
  • On completion, use a distinct state change, a sound, a highlight, a toast, so users who switched tabs know the result is ready.

2. The "Regenerate" Trap — Destroying User Work With One Click

Nearly every AI product has a "Regenerate" button. Nearly every one of them gets it wrong.

What we see in real products:

A content platform where users can generate articles from prompts. After the AI produces a draft, the user spends 10 minutes editing it, adjusting tone, fixing facts, adding their voice. Then they click "Regenerate" expecting a fresh alternative. The AI replaces everything. Their edits are gone. No undo. No version history. No warning.

We flagged this as a P0 critical issue with the note: "Regenerate is vague and can be destructive. Users can lose edits unknowingly if a regenerate replaces content without preserving versions."

Why this matters:

"Regenerate" sounds harmless. But it means different things in different contexts: "try again from scratch", "improve what's here", "give me an alternative", or "undo my edits and start over." Users cannot tell which one your product means until they have already lost their work.

How to fix it:

  • Never overwrite user-edited content without explicit confirmation. If the user has modified the AI output, treat regeneration as a new version, not a replacement.
  • Offer "Regenerate" as "Generate alternative" and show it alongside the current version, not instead of it.
  • Keep a version history. Even just "Version 1 (AI generated) / Version 2 (you edited) / Version 3 (AI alternative)" gives users confidence to explore.
  • If you must replace content, show a diff or at least a "Your previous version" recovery option.
  • Consider offering targeted regeneration: "Regenerate just this paragraph" instead of the entire output.

3. AI Features Scattered and Inconsistently Named

When a product adds AI capabilities over time, they tend to appear in different places under different names. The result is a product where users cannot build a mental model of what AI can do for them or where to find it.

What we see in real products:

A video editing platform with AI features spread across the interface: "AI Magic Edit tools" in one panel, "Transcript & Magic edit" in another tab, "Suggest Clips" as a button, and "Highlights clip generator" in a third location. Same underlying capability, AI helping you edit, but four different names and four different entry points.

We recommended: "Unify under a single AI entry point. Rename to 'AI editing shortcuts' with sub-sections: 'Clean up recording', 'Find highlights', 'Refine wording'. Use the same verb patterns everywhere."

A knowledge base tool with "Generate article", "Create with AI", and "Generate" appearing as different buttons across the same flow. Three labels. One action. Maximum confusion.

Why this matters:

Users do not read your product's documentation. They learn by scanning and clicking. If AI features use inconsistent language, users will either miss them entirely ("I didn't know it could do that") or hesitate to use them ("Is this the same thing I used before, or something different?").

How to fix it:

  • Audit every AI feature in your product and list them in a spreadsheet. Guaranteed you will find naming inconsistencies.
  • Pick one naming convention and apply it everywhere: "AI" as a prefix, or a consistent icon, or a dedicated section.
  • Create a single AI entry point or at minimum a consistent pattern. If AI can help with editing, that entry point should appear wherever editing happens, with the same label.
  • Test with a new user: ask them to list every AI feature in your product. If they miss more than one, your discoverability is broken.

4. Human vs AI Paths That Compete Instead of Complement

When you add AI to an existing workflow, you create a fork: the human path and the AI path. Most products handle this fork badly.

What we see in real products:

A live Q&A moderator tool with two side-by-side buttons: "Reply" and "AI Reply." Both the same size. Both the same visual weight. Both competing for attention during a live event when the moderator needs to act in seconds.

We flagged: "Two peer buttons (Reply and AI Reply) split attention. Simpler decision path improves speed under pressure."

The fix is not "make AI Reply bigger", it is rethinking the flow. The AI should draft automatically in the background. The moderator sees the draft and either sends it (one click), edits it, or ignores it and writes their own. The human path and AI path converge instead of competing.

Every decision point you add costs time and mental energy. "Should I use AI or do it myself?" is a question your users should not have to answer every single time they take an action. The best AI features are invisible defaults that users can override, not parallel paths that force a choice.

How to fix it:

  • Default to AI-assisted rather than AI-or-human. Let AI draft automatically, then let users accept, edit, or discard.
  • If you must show both paths, make the AI path visually primary and the manual path secondary (not equal weight).
  • Consider progressive disclosure: show the AI suggestion first, with "Write your own instead" as a secondary action.
  • Remove the word "AI" from the interface where possible. Users do not care whether the draft came from AI or a template. They care whether it is good.

5. AI Output Is a Wall of Text Nobody Will Read

Large language models produce text. That is what they do. But dumping a wall of generated text into a product interface is not a UX decision, it is an abdication of one.

What we see in real products:

An AI book recommendation tool where the AI returns numbered paragraphs explaining why each book was chosen. The explanations are thorough and well-written. They are also completely unreadable on a mobile screen where the user just wanted to know which book to read next.

A financial chat builder where the AI generates a multi-step execution plan mixing user-meaningful steps with technical internal steps. The plan is correct. It is also incomprehensible to anyone who is not a developer.

We consistently recommend: "Replace text-heavy AI output with structured cards, scannable summaries, and progressive disclosure."

Why this matters:

Users interact with AI output differently than they interact with content they chose to read. They are scanning for an answer, not reading for comprehension. If your AI produces five paragraphs when the user needed one sentence, you have wasted the user's time and the model's tokens.

How to fix it:

  • Structure AI output into scannable components: cards, bullet points, highlighted key phrases, collapsible sections.
  • Lead with the answer, then offer the explanation. "Use your Visa card for this purchase (saves $12)" is better than three paragraphs about reward point calculations followed by the recommendation.
  • For complex AI output (execution plans, multi-step processes), separate the "what will happen" summary from the technical details. Show the summary by default, let users expand for detail.
  • Set maximum lengths for AI output sections and enforce them. If your model generates 500 words, your UI should display a 50-word summary with "Show full analysis" underneath.
  • Use formatting that the interface controls, not the model. Do not rely on the LLM to produce well-structured HTML. Parse the output and render it in your own components.

6. No Clear Next Action After AI Output

AI generates something. Now what? In a surprising number of products, the answer is unclear.

What we see in real products:

An AI trading strategy builder where the user describes a strategy in natural language, the AI generates a backtest plan, simulates it, and shows results. The results screen shows performance metrics. But: "State transitions after confirmation (submitted, pending, succeeded, failed) are unclear. The user ends up checking History/Portfolio without an explicit completion narrative or next best action."

A content generator where after article generation there is "no review/confirm moment (title, location, template type, key settings) and no obvious next best action besides Publish, which may be too early."

Why this matters:

AI output is not the end of the user's journey, it is the middle. The user asked AI for help in order to do something else: publish an article, execute a trade, respond to a question, edit a video. If the transition from "AI gave me output" to "now I act on it" is unclear, the AI feature feels like a dead end no matter how good the output is.

How to fix it:

  • Every AI output screen needs a primary action button that answers "what do I do with this?" Make it specific: "Add these 4 tasks to my list", not "Continue." "Send this reply", not "Done."
  • Show a clear transition state: "AI suggested these clips. Select the ones you want to use and we'll create them." Not just a list of clips with no instruction.
  • If the AI output needs review before action, build that review step explicitly. Show what will change, what the user is committing to, and what happens next.
  • For long AI outputs, provide a "Quick summary" at the top with the recommended next action, then the full output below for users who want detail.

7. AI Error States Are Completely Missing

Every AI product will fail. Models time out. Rate limits get hit. Content filters trigger. Network connections drop. And in the vast majority of products we audit, none of these failure states have any designed UI.

What we see in real products:

A moderator tool where AI reply generation can fail due to timeout, error, or content being blocked. The designed UI for this scenario: nothing. No error message, no fallback, no indication that anything went wrong. The moderator assumes the AI is still "thinking" and keeps waiting.

A brain dump tool where the AI organization step can fail or return low-confidence results. The UI: "We couldn't organize this automatically", but with no recovery path, no option to retry, no option to manually organize instead.

A healthcare chatbot where the chat overlay fails to load entirely. The patient sees a blank panel with no explanation and no alternative way to reach their care team.

In every case, the product team designed the happy path (AI works perfectly) and forgot the unhappy paths (AI fails, partially fails, or returns low-quality results).

Why this matters:

AI failure is not an edge case. It is a certainty. Models fail more often and more unpredictably than traditional API calls. If your product has no UI for "AI could not generate a response," your users will encounter a blank screen or an infinite spinner and lose trust immediately.

How to fix it:

  • Design three AI states as a minimum: loading, success, and failure. If you have not designed all three, your AI feature is not done.
  • For failure states, always provide: (1) what went wrong in plain language, (2) a retry button, (3) a fallback to the manual path. "We couldn't generate a draft. Try again or write your own reply."
  • For partial failure (AI returns something but it is low quality), provide a quality signal and an easy path to regenerate or edit.
  • For total unavailability (AI service is down), degrade gracefully. The product should still work without AI, it just works less efficiently. Show: "AI suggestions are temporarily unavailable. You can still reply manually."
  • Log AI failures and surface them to your team. If 5% of AI requests fail and you have no error UI, 5% of your users are having a terrible experience that you cannot see in your analytics.

8. The AI Draft Lifecycle Nobody Designs For

AI generates a draft. The user does not act on it immediately. What happens next? In most products, the answer is: nobody thought about it.

What we see in real products:

A moderator tool during a live event. The AI generates a suggested reply. The moderator gets distracted by another question and navigates away. When they come back, is the draft still there? Has it been discarded? Can they recover it? Nobody designed for this.

Same tool, different scenario: another moderator sends a manual reply to the same question while the first moderator's AI draft is still pending. Now there is a conflict. The product has no UI for "someone else already answered this question while you were reviewing the AI draft."

And another: a question gets deleted or hidden by an admin while the AI is still generating a draft for it. The AI finishes. The question no longer exists. What does the moderator see?

We found four distinct draft lifecycle states that most products ignore:

  1. Draft exists but user navigated away, is it preserved? How do they find it?
  2. Draft conflicts with another user's action, another person answered/edited/deleted while you were reviewing the AI draft.
  3. Draft becomes stale, context changed (new information, price update, status change) while the AI draft was pending.
  4. Multiple drafts exist, user regenerated multiple times. Which version is current?

Why this matters:

Traditional product design assumes the user creates content and the content is theirs. AI content is different: it is generated speculatively, it may never be used, it can become stale, and multiple versions may exist simultaneously. If your product does not account for this lifecycle, users will lose work, send stale information, or encounter confusing states.

How to fix it:

  • Treat AI drafts as first-class objects with a clear status: "Draft ready", "Sending", "Sent", "Discarded." Show this status on the item the draft relates to, not just in the composer.
  • Auto-save AI drafts. If the user navigates away, the draft should persist and be recoverable. Show "Draft ready" status on the item when they return.
  • Handle conflicts explicitly. If someone else acts on an item while an AI draft exists, show a banner: "This question was already answered by [name]. You can still send an additional reply or discard the draft."
  • For stale drafts, either auto-refresh the draft with updated context or show a warning: "This draft was generated 5 minutes ago. The conversation has changed since then. Regenerate?"
  • If users can generate multiple drafts, let them browse versions: "AI draft 1 / 2 / 3" with a simple switcher.

The Common Thread

All eight patterns share one root cause: teams design AI features for the happy path and forget everything else.

The AI generates perfect output instantly. The user reads it, loves it, and takes the exact next action the designer imagined. Nobody navigates away. Nobody regenerates. Nobody encounters an error. Nobody is confused by the button label.

That is not how real products work. And the gap between that ideal and reality is where users lose trust, abandon features, and decide your AI is not worth the effort.

The fix is not more AI. It is more UX.
